This application is to be completed in full by all camp staff and volunteers for any position involving the supervision, teaching, or custody of campers. Information will be treated as confidential and is needed to help us provide a safe environment for children and youth who participate in our programs.
 
Please remember to sign and date your application. If you are 17 years of age or younger, a parent or guardian will also need to sign this application before a notary.

Reference

Please provide the names of three individuals (not relatives) who have known you for five years (5) or more and who can provide a reference for you. If you are under the age of 18, you may use the name of a parent and/or teacher.

If possible, please include at least one reference from someone in Bethel Evangelical Baptist Church. All people listed as references should be informed that you have listed them. References that are acceptable are limited to the following:
 
  • Former or present pastor
  • Long-time friend (minimum of 5 years)
  • One parent (for minors)

  • Church member (who has sufficient strength of relationship to comment on the individual’s personal habits and character)

Verification of Employment and Reference Authorization
I authorize former and present employers, and professional, work, and personal references listed in the application and any other individuals I may name, to give Bethel Evangelical Baptist Church or its designees any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release such parties from all liability for any damages that may result from furnishing such information to Bethel Evangelical Baptist Church.  I also authorize Bethel Evangelical Baptist Church to provide truthful information concerning my employment with it to future employers, and I agree to hold it harmless for providing such information.
Anti-Harassment Policy
It is our policy that employees and others acting on Bethel Evangelical Baptist Church’s behalf are entitled to respectful treatment in the workplace. Being respected means being treated honestly and professionally, with your unique talents and perspectives valued. A respectful workplace is about more than compliance with the law. It is a working environment that is free of inappropriate behavior of all kinds. We are committed to providing a workplace in which the dignity of every individual is respected. Each of us should understand that incidents of harassment and inappropriate behavior will not be tolerated at Bethel Evangelical Baptist Church.

A. Harassing Conduct
Harassment is unwelcome conduct toward an individual because of his or her race, color, sex, age, sexual orientation, religion, national origin, disability, or any other legally protected status, when the conduct creates an intimidating, hostile or offensive work environment that causes work performance to suffer or negatively affects job opportunities. Examples of harassment that may violate the law and will violate this policy include:
  • Oral or written communications that contain offensive name-calling, jokes, slurs, negative stereotyping, or threats. This includes comments or jokes that are distasteful or targeted at individuals or groups based on race, color, sex, age, sexual orientation, religion, national origin, disability, or any other legally protected status.
  • Nonverbal conduct, such as staring, leering and giving inappropriate gifts.
  • Physical conduct, such as assault or unwanted touching.
  • Visual images, such as derogatory or offensive pictures, cartoons, drawings or gestures. Such prohibited images include those in hard copy or electronic form.
B. Sexual Harassment
"Sexual harassment" means unwelcome sexual advances, unwelcome requests for sexual favors, unwelcome physical contact of a sexual nature or unwelcome verbal or physical conduct of a sexual nature. Sexual harassment includes conduct directed by a person at another person of the same or opposite gender. Unwelcome verbal or physical conduct of a sexual nature includes but is not limited to the deliberate, repeated making of unsolicited gestures or comments of a sexual nature; the deliberate, repeated display of offensive sexually graphic materials which is not necessary for business purposes; or deliberate verbal or physical conduct of a sexual nature, whether or not repeated, that is sufficiently severe to interfere substantially with an employee's work performance or to create an intimidating, hostile or offensive work environment.
C. Inappropriate Behavior
Our goal is to have a work environment where we all treat each other respectfully and professionally. Any unprofessional or disrespectful behavior, even If It does not rise to the level of "harassment," interferes with that goal and will not be tolerated. Bethel Evangelical Baptist Church reserves the right to respond to inappropriate behavior even where no one has complained or indicated they have been offended. 
D. How to Report a Violation
Do not assume that Bethel Evangelical Baptist Church is aware of the problem. It is your responsibility to bring your complaints and concerns to our attention so that we can help resolve them. Immediately report suspected violations of this policy to the pastor or the supervisor/ministry leader.
E. Investigation and Response
If you report a complaint of harassment or inappropriate behavior, we will investigate your concerns. Where there has been a violation of policy, we will take appropriate action to try to avoid future violations. In appropriate cases, disciplinary action (up to and including termination) will be taken against those violating the Anti-Harassment Policy. We will inform parties about the status of reviewing their complaints. To respect the privacy and confidentiality of all people involved, we might not share specific details of the discipline or other action taken.
F. Management Responsibility
Every supervisor and manager is responsible for ensuring that Bethel Evangelical Baptist Church provides a workplace free of harassment and inappropriate behavior and that complaints are handled promptly and effectively. With the assistance of Human Resources, management must inform their employees about the policy, promptly investigate allegations of harassment, take appropriate disciplinary action, and take steps to assure retaliation is prohibited.
G. Prohibition of Retaliation
This policy strictly prohibits any retaliation against an employee or other person who reports a concern about harassment or other inappropriate behavior.
H. Application
This applies to all employees and to anyone else doing business for or with Bethel Evangelical Baptist Church. This includes church attendees, customers, vendors, suppliers and contractors. It also applies to all locations and situations where Bethel Evangelical Baptist Church business is conducted and to all Bethel Evangelical Baptist Church-sponsored social events.
Discipline Policy
Employees and volunteers may be subject to discipline for many reasons, including performance failure, misconduct, inappropriate behavior, and failure to comply with company policies, including the Code of Conduct and other policies. This list is not all inclusive, but simply provides examples o conduct that may result I n discipline up to and including termination. We reserve the right to discipline any employee / volunteer and to determine the particular discipline to be imposed in any given case including, but not limited to, disciplinary actions such as verbal warning, or suspension, in any order, or termination of employment.
Child and Youth Abuse Prevention Program Acknowledgment
I have received a copy of the BETHEL EVANGELICAL BAPTIST CHURCH's Child and Youth Abuse Prevention Program. I understand it is my responsibility to become familiar with and adhere to the information contained herein. I understand that these policies are the property of the BETHEL EVANGELICAL BAPTIST CHURCH.

These guidelines have been designed to guide and assist you when working with minors. The information establishes general practices and guidelines and should not be construed in any way as a contract of employment or continued employment. BETHEL EVANGELICAL BAPTIST CHURCH reserves the right to make changes in the content or application of this program and to implement those changes with or without notice.
Authorization and Request for Criminal Records Verification and Fingerprint Information
I authorize Bethel Evangelical Baptist Church to obtain and/or request information about my criminal history and fingerprints for any entity chosen specifically for conducting this search, to release information regarding any record of charges or convictions contained in its files, or in any criminal file maintained on me, whether said file is a local, state, or national file, and including but not limited to accusations and convictions for crimes committed against minors, to the fullest extent permitted by city, county, state, and federal law. I do release said entities from all liability that may result from any such disclosurere made in response to this request. I may revoke this request at any time, but that revocation must be in writing and give 30 days’ notice of same.
Disclosures
All information in this application is held strictly confidential by the Bethel Evangelical Baptist Church Staff. Answering yes to any of the questions may not necessarily preclude your involvement in ministry here at Bethel Evangelical Baptist Church.    
I understand and agree that, if hired, my employment is for no definite period and may, regardless of the date of payment of my wages and salary, be terminated at any time without prior notice and without cause.”
I understand that Bethel Evangelical Baptist Church reserves the right, to the extent permitted by law, to require drug and alcohol screening tests of an applicant or an employee either prior to employment or anytime during employment, and I hereby give my consent to any such tests.  I consent to the release of the results of any such tests to Bethel Evangelical Baptist Church or its designees.  I release Bethel Evangelical Baptist Church and its designees from any and all liability and damages which may result or arise from any drug and alcohol screening tests or the provision of information in connection with such tests.
I understand that neither acceptance of this application nor the subsequent entry into any type of employment relationship with Bethel Evangelical Baptist Church creates an actual or implied contract of employment. I understand that, if I accept employment with Bethel Evangelical Baptist Church of Delray Beach, it will be on an at-will basis. This means that wither Bethel Evangelical Baptist Church of Delray Beach or I have the right to terminate the employment relationship at any time, for any reason, with or without cause.
